Our approach

We begin by understanding the participant’s goals, communication preferences, routines, culture, strengths and current support network. We then agree on clear responsibilities, safeguards and review points through the service agreement and support plan.

Support is adjusted as needs, goals and circumstances change. Where appropriate and consented to, we collaborate with families, nominees, clinicians, coordinators and other services to promote continuity and reduce unnecessary duplication.

NDIS funding information

Community nursing may be funded through Core or Capacity Building supports where the need is disability-related. Clinical eligibility and scope are assessed before service commencement. Important: Plan funding and claiming rules are participant-specific. We verify the relevant support category and service arrangement before delivery.
