Our approach

We begin by understanding the participant’s goals, communication preferences, routines, culture, strengths and current support network. We then agree on clear responsibilities, safeguards and review points through the service agreement and support plan.

Support is adjusted as needs, goals and circumstances change. Where appropriate and consented to, we collaborate with families, nominees, clinicians, coordinators and other services to promote continuity and reduce unnecessary duplication.

NDIS funding information

Accommodation and tenancy assistance may be funded as a capacity-building support. It does not include rent, utilities, property purchase or SDA housing. Important: Plan funding and claiming rules are participant-specific. We verify the relevant support category and service arrangement before delivery. We can discuss your goals, funding and service needs before you decide. 07 0000 0000 info@capstonecare.com.au
